If you choose to homeschool as a nonchartered, nonpublic (formerly known as an "-08") school in Ohio, you can use the forms below to keep and report the required records. HSLDA members can learn more about this homeschooling option here.

Ohio Nonchartered Nonpublic School Report of Attendance

Use this form to report student attendance at a nonchartered, nonpublic school to the treasurer of the local school board. The report of attendance must be made within the first two weeks of the beginning of the school year. This form does not include a reference to HSLDA.
